If you have one of these coordination disorders:

  • Dysphasia
  • Dyspraxia

Then, when filling out your "Accommodation Request" form, you will need to attach the following document: "Occupational Therapist's Report".


The Speech Therapy Report

The speech therapy report is a document required by the Special Needs Office. It must meet certain criteria to be deemed acceptable when evaluated by the CEBP.

 

List of criteria to meet

The speech therapy report must be:

  • Typed,
  • In PDF format,
  • Written in French or English,
  • Dated within the last 3 years (if the assessment was conducted before the age of 18) or dated within the last 5 years (if the assessment was conducted after the age of 18),
  • Conducted with tests appropriate for the student's age and level of education and contain the quantitative results of these tests,
  • Describe and justify the requested accommodations.

In the absence of standardized tests for the evaluation of peripheral dysgraphia in an adult population, the evaluation of handwriting must objectively demonstrate an impairment that interferes with certain types of university exams. Therefore, it is necessary that the assessment report includes scores from writing tests (copying, dictation, written production, etc.) that validate a deviation from the norm.

If the requested accommodation involves the use of a computer, it should be demonstrated that typing provides a reduction in difficulties and better legibility compared to handwriting.
